Erythromycin

[Properties] This product is colorless and transparent capsule, the contents of which are white or almost white enteric coated erythromycin alkali pellets.

[Indications] It is mainly used for tonsillitis, pneumonia, scarlet fever, diphtheria, carriers, gonorrhea and other infections caused by G+ sensitive bacteria such as Staphylococcus aureus, Streptococcus pneumoniae and other Streptococcus genera. It can also be used for upper respiratory tract infection caused by influenzae (such as ear, nose, throat infection, etc.), skin and soft tissue infection caused by staphylococcus aureus, syphilis, intestinal amoebiasis, etc. This product can be used as the first choice for Legionnaires' disease, mycoplasma pneumonia, etc.
           

[Usage and Dosage] Oral administration, 1 ~ 2g per day for adults, divided into 3 ~ 4 times.

[Adverse Reactions] 1. Gastrointestinal reactions include diarrhea, nausea, vomiting, stomach cramping, gastric hypopnea, etc. The incidence is related to the dose size. 2. Allergic reactions include drug fever, rash, and eosinophilia, with an incidence of 0.5-1%. 3. Temporary AST transaminase elevation and occasional cholestatic hepatitis.

[Drug interaction] 1. Erythromycin can inhibit the metabolism of carbamazepine, leading to increased blood drug concentration of the latter and toxic reactions. 2. Erythromycin has antagonistic effect on chloramphenicol and lincomycin, so it is not recommended to use them together. 3. Erythromycin may lead to prolonged prothrombin time in patients taking warfarin for a long time, thus increasing the risk of bleeding, especially in elderly patients. The warfarin dose should be adjusted appropriately and the prothrombin time should be observed closely. 4. With the exception of dihydroxypropionophylline, erythromycin combined with xanthine can reduce liver clearance of aminophylline, leading to increased serum aminophylline concentration and/or increased toxic reactions. The decrease of aminophylline elimination was directly proportional to the peak value of erythromycin serum. Therefore, the dosage of xanthine should be adjusted during and after the same treatment course. 5. Discontinuation of bromase can increase the plasma concentration of the latter, even reaching toxic levels. Clinical and ECG monitoring is recommended, and the dosage of digoxin should be adjusted if necessary. 6. Combined use with cyclosporine: erythromycin can increase the levels of cyclosporine and creatinine in the blood. Monitoring of renal function, determination of plasma cyclosporine level, and adjustment of cyclosporine dosage during and after treatment are required when combined use. 7. Symptomatic ergotic poisoning, possibly accompanied by acronecrosis, has been reported due to the combination of erythromycin with ergotamine containing drugs or other ergot-derived vasoconstrictors.

The antibacterial spectrum of erythromycin is as follows: Common sensitive strains: Blood group A streptococcus, light streptococcus, streptococcus, pneumococcus, neisseria meningitidis, neisseria gonorrhoea diplococcus, whooping cough, generation of bacterium, catarrh mora's bacteria, corynebacterium diphtheriae, mononucleosis listeria, clostridium difficile, mycoplasma pneumoniae, sand holes, chlamydia, legionella, treponema pallidum, leptospira, campylobacter. The strains with changed susceptibility: Haemophilus influenzae, Bacteroides fragilis, Bacteroides cholerae, Staphylococcus aureus drug-resistant strains: Enterobacteriaceae, Pseudomonas.
